Intake by link, before the appointment
The patient answers in their own time, and you start knowing the goal, the restrictions and the routine. No filling in forms while they wait.
Notes with author and date
Every entry stores who wrote it and when. The history stays readable months later, including for whoever wrote it.
Documents with signature and verification
Prescriptions, certificates and forms come out of the appointment, get signed, and can be checked by public link. The patient does not need you to prove the document is yours.

Reception books and registers; clinical content is a different permission level, and access is logged.
